WebWe provide care and support to individuals with complex functional mental health needs aged over 65 years, or those of any age with a diagnosis of dementia. We are a multi-disciplinary team and individuals will have access and assessment provided by psychology, medics, mental health nurses and occupational therapists as required. WebThe Community Dental Service provides dental care to vulnerable patients who may find it difficult to go to a dental practice on the high street e.g. housebound people. You can be referred into this service by a healthcare professional like a dentist, GP or health visitor. You can also refer yourself or a family member by telephoning:
